Primary care designed around you

Imagine a place made just for you—mind, body and spirit. Where your care is made simple and your care team builds a plan unique to you. Welcome to Baylor Scott & White Health.

Having a primary care team that knows you is more important than you may think. Our goal is to keep you healthy. But if your care gets complicated, you want to know that you have access to a team of experts with a complete view of your health, so they can build a care plan around you.

Not only do you have access to a large network of providers for every need, but also the technology to make those relationships even stronger. And it’s easy to stay in touch. You can download the MyBSWHealth app and message your doctor whenever you want, schedule your next appointment, see your test results and more. It’s the care you need, when and where you need it.

And just like you can keep track of all your health records in one place, your network of providers at Baylor Scott & White can keep track of it, too. They can access your medical records and health history in real time and communicate with each other—resulting in a connected care experience where everyone is on the same page.

Because that’s what getting Better is all about—care that’s easy and centered on you. Thank you for choosing us to help you get Better—and stay that way.

To-do list

Take an active role in your care.

Getting started

Your Primary Care Provider (PCP) is your first partner in wellness.

Just as you’d be open and honest with your partner, loved ones or family members, you’ll want to establish and nurture the same kind of relationship with your provider. Ask yourself how well your primary care physician knows you today. If you’re already close, that’s great. If not, make a plan to get better acquainted and talk about your goals—even (and especially!) if you’re healthy and just want to know how to stay that way.

Here are some tips to help you get started talking to your PCP:

Where to go for care

Find a care team, urgent care, walk-in clinic or emergency room near you.

Care team services

Here are some things that your PCP or APP can offer that may surprise you.

Care teams serve you Better

Get the care you need on your schedule, thanks to a patient-centered care team approach. Advanced practice providers (APPs) collaborate with your primary care physician to give you more—more flexibility in appointment scheduling and more collaboration in your care.

APPs can include clinical nurse specialists (CNS), nurse practitioners (NP) and physician assistants (PA). These nationally certified licensed healthcare providers have advanced education and training that prepares them to practice medicine collaboratively with your primary care doctor on your healthcare team.

As part of your unique plan, APPs can provide services like:

  • Diagnosing and treating medical conditions and injuries
  • Performing physical exams
  • Prescribing medications and other treatments
  • Ordering and interpreting lab tests, imaging studies like X-rays and other diagnostic testing
  • Performing annual exams (such as well-adult or well-child)
  • Treating acute illnesses and injuries
  • Educating and counseling you on maintaining a healthy lifestyle
  • Performing in-office procedures like sutures, joint injections and skin biopsies
  • Managing chronic conditions like diabetes, high blood pressure and respiratory diseases
