Home care may be recommended by your physician for conditions including:
- Post surgical needs
- New diagnosis
- Uncontrolled Diagnosis
- CHF, COPD, diabetes, anxiety, depression, dementia
- Changes in medication
- New Treatment
- Catheter, glucose meter, nebulizer, ostomies
- Wound care
- Strength and gait issues
- Stroke rehabilitation
Who qualifies for home care services?
Home Health services are for those recovering from illness, surgery or need assistance managing a chronic condition or pain from home.
To be eligible for home healthcare, a physician must certify that an individual is in need and is homebound.
“Homebound” does not mean bed-ridden, it means that a patient cannot currently leave the home due to illness or injury without “considerable and taxing effort”.

AccentCare
On August 1, 2017, Baylor Scott & White Health and Texas Home Health formed a joint venture—AccentCare—to deliver home health services to 53 counties from eleven locations: Brenham, College Station, Denton, Desoto, Fort Worth, Huntsville, Marble Falls, McKinney, Taylor, Temple, Waco, and Weatherford.
